New Procedures Announced for Replacement COVID-19 Vaccination Cards

The Kentucky Department for Public Health has changed the procedure for obtaining replacement proof of COVID-19 vaccination.  CDC and the state health department advise that new, replacement COVID-19 vaccine cards (the small white cards people receive from the vaccine provider at the time of vaccination) no longer be given to vaccine recipients who have lost or misplaced their original vaccine cards.  Instead, providers who administered the COVID-19 vaccine are asked to provide replacement proof of COVID vaccination by accessing a new feature in the Kentucky Immunization Registry.   An example of such a report that can be obtained from the immunization registry is attached.

In the event someone who has been vaccinated for COVID-19 loses or destroys their small white CDC COVID-19 vaccine card, they can get a KYIR COVID-19 vaccine report by doing the following:

  • If the person received their vaccine through LFCHD, they can phone our COVID-19 Call Center at 859-899-222 or send an email to covidcardreplacement@lfchd.org indicating they are in need of replacement proof of vaccination.
  • If the person received a vaccine from a provider other than LFCHD, the provider can generate a COVID-19 report through KYIR. Click here for the instructions.
